Additional Documents Required

The following items need to be submitted along with the application in order for us to process it:

• Grade Reports/School Records from previous 2 years
• Birth certificate or proof of birth date
• Writing sample for Grades 2-1
• Mission statement and purpose of mission organization, if applying for mission status
• IEP, if student has been assessed or placed in any special education programs
